Advocacy & Awareness

Advocacy services include advocating for caregivers rights and keeping caregivers abreast of pending legislation and breaking news. Excursions to visit legislators and state representatives are scheduled annually to shed light on the pressing issues.

Support Coordination

The Support Coordinators work with individuals with developmental disabilities in creating and developing life plans. Currently, Caregivers of New Jersey supports the 1,850 individuals living in developmental centers who wish to be transitioned into the community.

The Support Coordination model is consumer-driven and self-directed by individuals and their families. It is recognized that moving people into the community does not in and of itself make people a part of the community. Therefore, Support Coordinators link individuals with provider agencies, natural supports and other community resources. Support Coordination assists and empowers the individual and their team through a process but allows them to remain in control of their plan by holding individuals and families responsible for choosing both the services and providers they would like. This model involves helping individuals and families develop an Essential Lifestyle Plan, access services identified in the plan, arrange for payment and oversee and help to revise the care plan when the individual and their family needs or would like a change.
